The document provides an evidence-based summary of TOC resources in one central location. The document focuses on applying the Joint Commission of Pharmacy Practitioners (JCPP) Pharmacists’ Patient Care Process (PPCP) to the provision of care transitions services. The PPCP approach maximizes a pharmacist’s role within health care delivery models that are collaborative and team-based.

The Transitions of Care Case Examples were designed as a members-only resource for pharmacists and student pharmacists to improve transitions of care (TOC) services across a multitude of health care settings. The document addresses pharmacists’, student pharmacists’, and pharmacy technicians’ roles in transitions of care services within a community hospital, an outpatient health system pharmacy, an ambulatory care clinic, a long-term care facility, and a community pharmacy.

This resource was created by the American Pharmacists Association Transitions of Care (TOC) Special Interest Group (SIG).

The resource is intended to be used as a reference only. The examples in this document should not be interpreted as guidelines or best practices.
