Blueprint element #9: Implement and improve
The implementation phase will turn a concept and negotiations into reality. After planning the initial implementation requirements, the practice should be ready to begin offering pharmacists’ services. Following the implementation plan will allow all relevant parties to understand the process.
As implementation evolves and communication is strengthened, evaluating how things are going or can the partnership can perform better is the final piece to assess and improve upon the services offered by the pharmacist(s). Providing progress reports that assess for gaps in the practice site in which the pharmacist is underutilized or could fulfill an unforeseen gap can be helpful in informing decision-makers. Success will rely on continual communication with key decision-makers and putting a process in place that measures success.
