Blueprint element #2: Think about the team structure

The pharmacist’s practice model will serve as the foundation on which team-based partnerships can be built. Once the practice model is established, the pharmacist can begin to think about the type of team they would work best with to deliver patient care services. Team-based models of care are formed when health care providers work collaboratively to achieve patient-centered goals.
