Transitions Of Care Disease State Checklists

The Transitions of Care Disease State Checklist Tool is an APhA member-only resource created for pharmacists, pharmacist extenders(s), or any healthcare personnel engaging in transitions of care services. Each checklist can help guide and ensure effective transitions for patients at high risk for readmission such as patients diagnosed with acute coronary syndrome, chronic obstructive pulmonary disorder, coronary artery bypass graft surgery, community acquired pneumonia, heart failure or stroke.
