Patient-Centered Deprescribing in Older Adults Certificate Training Program

This comprehensive training program equips pharmacists with the knowledge and skills to confidently and compassionately deprescribe medications in alignment with patient-centered care principles. Through interactive modules, case-based learning, and practical tools, participants will learn to evaluate the appropriateness of medications based on individual patient goals, prognosis, beliefs, and values. By the end of the course, learners will be empowered to lead deprescribing efforts that reduce polypharmacy risks, enhance quality of life, and support meaningful patient outcomes. This course is eligible for BCGP recertification credit.

Learning Objectives & Module Details
Module 1: Exploring the Role and Value of Deprescribing in Clinical Practice

At the completion of this Practice-Based activity, participants will be able to:

  1. Discuss the role of deprescribing when managing medication overload.
  2. List the five steps of deprescribing.
  3. Identify barriers to deprescribing clinical practice implementation.
Module 2: Developing Successful Deprescribing Plans

At the completion of this Practice-Based activity, participants will be able to:

  1. Name the four parameters of the Model for Rational Prescribing.
  2. Describe how understanding a patient’s medication-related goals of care is important to developing a successful deprescribing plan.
  3. Identify nonpharmacological interventions as opportunities to engage patients in deprescribing, build trust, and align goals of care.
Module 3: Determining Deprescribing Eligibility for Statins, Antihypertensives, and Antihyperglycemic Medications

At the completion of this Practice-Based activity, participants will be able to:

  1. Review evidence-based literature for the initiation and discontinuation of statins, antihypertensives, and antihyperglycemic medications.
  2. Describe the deprescribing eligibility of these medications based on a patient’s goals of care, prognosis, beliefs, and values.
  3. Apply deprescribing eligibility criteria to a patient case.
Module 4: Determining Deprescribing Eligibility for Benzodiazepines, Antidementia Medications,
and Antipsychotics

At the completion of this Practice-Based activity, participants will be able to:

  1. Review evidence-based literature for the initiation and discontinuation of benzodiazepines (BZDs), antidementia medications, and antipsychotics.
  2. Describe the deprescribing eligibility of these medications based on a patient’s goals of care, prognosis, beliefs, and values.
  3. Apply deprescribing eligibility criteria to a patient case.
Module 5: Engaging Patients and Their Caregivers Through Deprescribing Communication Techniques

At the completion of this Practice-Based activity, participants will be able to:

  1. List the five actions of the FRAME deprescribing communication map.
  2. Describe ways to manage cognitive dissonance.
  3. Review publicly available patient-focused deprescribing resources.
Module 6: Implementing Deprescribing into Clinical Practice

At the completion of this Practice-Based activity, participants will be able to:

  1. Develop strategies to overcome deprescribing implementation clinical practice barriers.
  2. Compare approaches for implementing clinical deprescribing services.
  3. Discuss best practices for communication and interprofessional collaboration when deprescribing.
  4. Outline important documentation considerations when deprescribing.
Module 7: Implementing Deprescribing Initiatives within Integrated Delivery Networks, Health Systems, and Pharmacies

At the completion of this Practice-Based activity, participants will be able to:

  1. Describe the function and purpose of the CancelRx National Council for Prescription Drug Program script standard for reducing inadvertent medication refills.
  2. Recall challenges associated with the rollout of the CancelRx standard from the State of Connecticut CancelRx Collaborative.
  3. Discuss opportunities for integrating deprescribing pathways/algorithms within electronic medical records and health information exchanges.
  4. Explain how AI can support deprescribing activities.
Module 8: Sustaining Deprescribing in Practice

At the completion of this Practice-Based activity, participants will be able to:

  1. Appraise currently available literature on strategies to sustain deprescribing in practice.
  2. Compare and contrast the effectiveness, quality, and resource needs of deprescribing informatics tools and strategies.
  3. Practice drafting a deprescribing-focused business plan.
  4. Apply deprescribing sustainment strategies to a population-level cohort case.

How to Obtain a Certificate of Completion

To earn a Certificate of Completion for APhA’s Increasing Access to Hormonal Contraceptive Products, participants must meet all the following requirements:

  • Complete eight self-study modules, each of which consists of an interactive eLearning module, an assessment, and a module evaluation.
  • Achieve a passing grade of 70% or higher on each module assessment.
  • Credit must be claimed for each module in order to obtain credit.
  • Participants will need to have a valid APhA (pharmacist.com) username and password, as well as a CPE Monitor account to claim credit. After credit has been claimed, please visit CPE Monitor for your transcript.
  • The certificate of completion will be available online upon successful completion of all eight module activities on the participant’s My Training page.
  • APhA CPE policy provides you with two opportunities to successfully complete a CPE assessment. Please note that you will not be permitted to submit an assessment a third time. The current policy of the APhA Education Department is not to release the correct answers to any of our CPE tests. This is intended to maintain the integrity of the CPE activity and the assessment.
Accreditation Information

The American Pharmacists Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. APhA's Patient-Centered Deprescribing in Older Adults Certificate Training Program is approved for a total of 8.0 contact hours of continuing pharmacy education (CPE) credit (0.8 CEUs).
