Since the approval of the first direct-acting oral anticoagulant (DOAC) in 2010, prescriptions for agents from this drug class to manage chronic conditions have increased dramatically. Compared with the vitamin K antagonist (VKA) warfarin, DOACs offer more predictable dosing, less need for laboratory monitoring, and fewer drug–drug and drug–food interactions. Originally approved only for prevention of stroke or systemic embolism (SE) in nonvalvular atrial fibrillation (AF), DOACs are now prescribed for several FDA-approved indications, including venous thromboembolism (VTE) treatment, VTE prophylaxis in acutely ill medical patients and orthopedic surgery, coronary heart disease (CHD), and peripheral artery disease (PAD). As management of DOAC therapy continues to evolve at a rapid pace, pharmacists may struggle to stay abreast of recent evidence and practice changes. This article reviews recent developments in DOAC use for chronic conditions and provides guidance on how pharmacists can engage in patient-centered management of anticoagulation therapy.

To claim credit

2. Log in to your APhA account, or register as a new user.
3. Select “Enroll Now” or “Add to Cart” (click “View Cart” and “Check Out”).
4. Complete the assessment and evaluation.
5. Click “Claim Credit.” You will need to provide your NABP e-profile ID number to obtain and print your statement of credit.
