The evolving nature of science: APhA’s enduring commitment to evidence-based practice

How the American Pharmacists Association champions science, integrity, and adaptability in health care

In a world where information is instantaneous and opinions are plentiful, the American Pharmacists Association (APhA) stands as a steadfast advocate for science-driven health care. Pharmacists occupy a unique and critical role in the health care ecosystem, bridging the gap between research and patient care. At the heart of APhA’s mission is a commitment to continually evaluating scientific evidence, ensuring that decisions are rooted in the best available knowledge—while maintaining the humility to recognize that science itself is a living, evolving pursuit.

APhA’s commitment to evidence-based decision making

From medication counseling to public health initiatives, pharmacists are trusted voices in their communities. APhA’s dedication to evidence-based decision making ensures that pharmacists’ recommendations are not merely tradition or anecdote but are guided by rigorous scientific findings. This commitment is not static; rather, it requires ongoing critical appraisal of new research, guidelines, and technologies.

As more patients are treated with specific therapies and therapeutic options advance, we learn new things. For example, when clopidogrel (Plavix) was first brought to market, we didn’t understand why some people responded to the therapy and others did not. It was only after significant continued research that a genetic marker was identified that helped us know for whom the drug should and should not be prescribed. Science guided us to better clinical decisions.

Taking bold stands in support of science

Throughout its history, APhA has not shied away from making bold, sometimes challenging, decisions in support of sound science. Whether advocating for widespread vaccination, supporting measures to curb opioid misuse, or addressing misinformation about COVID-19 treatments, APhA’s leadership is rooted in the best available data.

This willingness to take a stand is not just about following consensus—it’s about standing up for public health, even when doing so is controversial or politically fraught. By publicly supporting vaccination campaigns and evidence-based prescribing, APhA reinforces the profession’s role as a trusted source of truth and a bulwark against pseudoscience.

Science is not static: Embracing new evidence

One of the most profound strengths of science is its capacity for self-correction. As new evidence emerges, previously held beliefs may be challenged or refined. APhA recognizes that the scientific process is not about certainty but about reducing uncertainty through continuous inquiry and rigorous analysis.

This openness to new information is not a weakness; it is a strength. APhA encourages pharmacists to remain intellectually curious and adaptable, willing to update their practices in light of robust new data. When new studies are published or guidelines are revised, APhA facilitates the dissemination and discussion of these findings, promoting a culture where change is embraced when justified by evidence. Because, at the end of the day, our patients deserve care that reflects the very best of what science has to offer.

Not all evidence is created equal: Understanding the hierarchy of scientific research

When interpreting scientific literature, it is crucial to acknowledge that not all studies carry the same weight. APhA educates its members about the hierarchy of scientific evidence, which places systematic reviews and randomized controlled trials at the top, followed by cohort studies, case-control studies, and, finally, case reports and expert opinion.

Case reports, which document observations from individual patients or clinical experiences, play a valuable role in identifying novel phenomena or rare adverse effects. However, they are inherently limited by small sample size and the absence of control groups, making them less reliable for establishing causality or generalizing findings to broader populations.

APhA teaches pharmacists to appreciate the insights that case reports can provide—such as signaling the need for further investigation—while also understanding their limitations. This nuanced approach is essential to avoid overreacting to early signals or prematurely dismissing critical new developments.

Fostering critical appraisal and ongoing dialogue

The recent questions raised about acetaminophen use in pregnancy and a possible link to autism struck a strong nerve for both patients and health care professionals. Having others report on the literature, particularly those without a health care background, can be challenging. Further juxtaposed political opinions raise emotional stakes even more.

Pharmacists’ education and training are rooted deeply in science. APhA’s commitment to science extends beyond the passive acceptance of published research. The association fosters an environment where critical appraisal is valued and open dialogue is encouraged. Pharmacists are trained to ask probing questions: Was the study well-designed? Were the results statistically and clinically significant? Do the findings align with the broader body of evidence?

Through continuing education, peer-reviewed publications, and professional forums, APhA provides pharmacists with the tools to rigorously assess the quality and relevance of research, ensuring that practice evolves in step with the best available evidence.

Conclusion: Science, integrity, and the future of pharmacy

APhA’s unwavering commitment to science is foundational to the trust pharmacists have earned with patients, colleagues, and communities nationwide. By taking bold stands rooted in evidence, remaining open to new and emerging data, and educating pharmacists on the strengths and limitations of various research types, APhA ensures that the profession remains both principled and adaptable.

As the health care landscape evolves and new challenges arise, APhA’s dedication to ongoing scientific evaluation will remain a guiding light—empowering pharmacists to deliver care that is not only innovative but also safe, effective, and grounded in the best that science has to offer. Our patients and the communities we care for deserve nothing less.
