Here are a list of digital files that need to be uploaded to the application website:
* Please note that Statements of Consent should be emailed directly to the Chapter Advisor and Experiential Coordinator for each to sign and date the document. Once signatures have been obtained, please scan the document and upload the digital copy to the Standing Committee Application - Section IV.
In addition, the following information is necessary to complete your application:
- The name and email address for the authors of your 2 letters of recommendation. Through the application system, authors will be contacted by APhA to upload their letter of recommendation by Friday, December 1, 2017 at 11:59pm PT. Applicants will receive status updates on their letters of recommendation. Please contact your authors as soon as possible. Letters of recommendation will not be accepted after the deadline.
- Complete an online personality test by visiting 16 Personalities (a Myers-Briggs type indicator) and report your results. If you have already completed Myers-Briggs, you can simply list your personality type without taking the test.
- If you have completed a Clifton StrengthsFinder Assessment, we would like to know your top 5 strengths. If you have not conducted an assessment, this is not necessary for submitting a National Standing Committee application. StrengthsFinder Assessments cost around $15-$20 and can be accessed online or through several of their books.
