Site Description: APSC is a cooperative of over 200 independent community pharmacies. Their Pharmacy Care Division (PCD) provides the resident with a unique opportunity to assist many pharmacies with the development and implementation of enhanced pharmacist care and disease state management programs. The resident's primary practice site will be Wheeler Pharmacy in Lexington. This site focuses in community pharmacy and medication therapy management services in patients' homes. At the corporate level, the resident will learn relevant government affairs issues that affect independent pharmacies, develop continuing education programs and facilitate conferences, create new disease state management programs, and precept 4th year PharmD students. At the pharmacy level, the resident will design, market, and implement a number of disease state management programs and clinical services in independent pharmacies. An enormous advantage in doing a residency at APSC versus an individual independent community pharmacy is in the variety. Residents have the opportunity to work with several independent pharmacies affiliated with APSC in addition to working with the supportive and very knowledgeable corporate office staff.
Services Offered: Each independent pharmacy affiliated with APSC has its own niche in pharmacy practice. One of the primary sites, Morehead Clinic Pharmacy, is located within a physician clinic and provides anticoagulation disease state management utilizing collaborative care agreements with physicians to monitor drug therapy. Other sites provide extensive diabetes education programs through pharmacists who are Certified Diabetes Educators. There are also pharmacies who specialize primarily in compounding services or home infusion. The pharmacy has functioned as a resident training site suince July 1999. The program follows a July 1-June 30 cycle. Site Description: Hartzell’s Pharmacy, established in 1968, is an independent family-owned pharmacy located in Catasauqua, Pennsylvania, which is located in the Lehigh Valley Area, approximately 90 minutes northwest of Philadelphia and 2 hours southwest of New York City. Hartzell’s Pharmacy/Wilkes University residency program is a PGY-1 pharmacy practice residency with an emphasis in community care, located in an independent community pharmacy setting. The resident will participate, develop and refine patient care services including immunizations, medication therapy management, health and wellness screenings and community outreach education. Disease state emphasis will include but is not limited to diabetes, immunizations, travel medicine, hypertension, dyslipidemia, and osteoporosis. Program elements also include practice management, marketing, and reimbursement strategies. The resident will have additional longitudinal experiences in an ambulatory care clinic and teaching responsibilities to Wilkes University, Nesbitt School of Pharmacy.
Services Offered: Clinical services offered at Hartzell’s Pharmacy include immunization and travel medicine clinics, medication therapy management, heath and wellness services, durable medical equipment, and respiratory therapy. Hartzell’s also offers medication services to assisted living facilities, and has a specialty compounding pharmacy, Catasauqua Compounding Pharmacy, within its facility. Several pharmacies have new designs containing private consultation areas to enhance pharmaceutical care services provided. The resident will primarily work from the new Patient Care Center located in Lexington at the Alexandria Drive site. He/she will be able to provide a variety of mobile services throughout the marketing area pharmacies such as diabetes screenings, immunization programs, and health and wellness programs. Many pharmacists at Kroger have received various certifications in disease state management including NIPCO certification programs, APhA’s Immunization Delivery training, and Cooper/Clayton Smoking Cessation facilitator’s training. The advantage to working with Kroger pharmacy is the opportunity to implement programs throughout the entire marketing area and potentially influence the practices of several different pharmacies throughout the marketing area.

This residency offers the opportunity to provide pharmaceutical care in the community pharmacy setting, and rotations will be adjusted to maximize the experience, taking into consideration the strengths, weaknesses and interests of the resident as well as the needs of the program. The Residency Learning System (RLS), developed by the American Society of Health-System Pharmacists (ASHP), is used as the format for this residency program.

Located in Fairfax, VA, within 10 miles of Washington, DC, the pharmacy provides dispensing services along the east coast, specializing in infertility, hepatitis C, Crohn's disease, oncology, and HIV/AIDS. The location opened in 2006. The resident will work closely with the Clinical Pharmacy Specialist to execute and expand clinical services for HIV/AIDS, Hepatitis C, and oncology. Site Description: The residency will center on the care patients receive from the Rite Aid pharmacy in Grafton adn Morgantown, West Virginia while several sites will be utilized. Grafton is a small town with a large population of 50 year old adn over patients. The majority of the patients from this pharmacy have Type I and Type II diabetes mellitus. The resident will have access to these patients to teach them about their disease and how to manage it properly. As a result of the program, the resident will be expeted to exhibit competency in the following areas: · Professionalism · Practice Management · Communication · Data Collection and Management · Clinical Problem Solving · Practice-based research/scholarship The main goal of the program is for the resident to create, apply, and manage drug therapy, provide patient care and develop disease-state management programs. For teaching purposes, the resident will be afforded administrative support. The resident will precept students in the experiential setting and as their interest's dictate will provide letures in required nonprescription drugs and therapeutics courses. In addition the resident will be involved in community outreach education centered on diabetes.
Services Offered: The residency will emphasize diabetes education, so the resident will be required to attend the Pharmacy Partners in Diabetic Care (PPDC) training in San Jose, California. When the resident returns from their training, they are scheduled to work within the pharmacy to begin their diabetic care center. Site Description: Ukrop's is a local supermarket chain with 27 stores in the area of greater Richmond, Fredericksburg, and Williamsburg, Virginia, nineteen of which have pharmacies. Currently, ten of the pharmacies have Wellness Centers, which are private patient concultation rooms each with a CLIA-waiver for patient testing. Six other pharmacies have private consultation centers for Wellness Center expansion. Ukrop's Pharmacy has been partnering wiht the Virginia Commonwealth University (VCU) School of Pharmacy since 1996 to enhance their pharmacy practice by providing pharmaceutical care. The goal of the Ukrop's Pharmacy programs is to include a focus on drug therapy monitoring, prevention of drug-related problems, disease state management, and wellness, including diet, exercise and disease prevention. Site Description: University Health Connection (UHC), located in the OSU College of Pharmacy, is one of four accredited community-care practice sites. The primary focus of this practice environment is to provide health and wellness services to faculty and staff of The Ohio State University. In addition to direct patient care services, the clinic connects faculty and staff to the benefits of the OSU health plan and other resources available on campus. UHC is a partnership between the OSU HealthPlan, Human Resources, Emergency Physicians, Inc., Student Health Services, the Department of Medicine, the College of Nursing and the College of Pharmacy and under the direction of the primary care network. The resident is part of an interprofessional practice team that includes physicians, nurses, pharmacists, massage therapists, reflexologists, social workers, and students from all disciplines. The resident is responsible for designing and conducting a practice-based research project. In addition, the resident will be encouraged to participate in ongoing projects and to assist with the development of new UHC services. UHC was a 2005 recipient of the ASHP Best Practices Award.
Services Offered: The resident will see patients along side the physician to make drug therapy recommendations based on patients’ co-morbid disease states and current medication regimen. Today, as a result, Uptown offers blood pressure screenings, cholesterol screenings, anticoagulation management, adult immunizations, comprehensive medication therapy management consultations (MTM’s), disease education, custom prescription compounding and flavoring, medical device training, and hormone replacement therapy management. The pharmacy serves as a rotation site for 4th year pharmacy students from The Ohio State University, Ohio Northern University, and the University of Cincinnati. Residents will be part of our pharmacy care team and therefore responsible for involvement in all pharmacy care activities as well as prescription dispensing. The resident will also develop skills in business administration, practice management, marketing, and student precepting. In addition, the resident will have the opportunity to participate in ongoing projects and assist with the development of new clinical services within the pharmacy and Westerville community. The resident will also provide medication consulting within a primary care physicians' office on a routine basis and participate in advocacy and association management with OPA.

In addition to the clinical program implementation and patient care duties, the resident will experience all aspects of pharmacy management from a Pharmacy Manager and District Manager level through the corporate office level. Additional areas of experience in pharmacy management include recruiting, pharmacy acquisition and third party billing. Residents will participate fully in the University teaching and learning program as well as rotate through traditional ambulatory care clinics. Site Description: This Post Graduate Year 1 residency focuses on: 1. Clinical practice, project management, teaching, and pharmacy operations. The resident will rotate through a variety of community/ambulatory care practices. Teaching activities and responsibilities are available for residents throughout the USC School of Pharmacy curriculum. Site Description: This community pharmacy practice residency provides residents with extensive experience in an advanced clinical practice setting along with opportunities to conceptualize, integrate, and transform accumulated experiences into skill and competence in providing patient care and advancing the practice of pharmacy. The program focus will be on the creation and advancement of self-sustaining, pharmacist-driven pharmaceutical care initiatives. Residents receive training in several different areas including anticoagulation, diabetes and endocrine, pulmonary, HIV, oncology, solid organ transplantation, pharmacy management, human relations, pharmacy administration, managed care, drug information and drug policy development, medication safety, and advancement of pharmacy practice. Residents are involved in education of UW pharmacy students in pharmacotherapy laboratory and clinical settings and present research, quality improvement, and pharmacy advanced initiative projects at local, regional, and national meetings. The opportunity exists for the community pharmacy resident’s experience to be tailored in order to facilitate further advanced opportunities focusing on pharmaceutical care program development, education, and/or management at UWHC.

Site Description: Valley View Clinical Pharmacists (VVCP) has been providing pharmaceutical care to the growing community of Monroe since 1981. The goal of the residency program is to produce an advanced clinical practitioner in a community-focused clinic pharmacy. VVCP is located within the Medalia Medical Center. This primary care clinic is staffed by nine family physicians, four internists, four mid-level practitioners, and three pediatricians. VVCP also is contracted to provide professional pharmacy services for the 38-acute care bed community hospital located directly across the street from the clinic. The hospital offers clinical opportunities in general medicine, critical care, and behavioral health. The clinic pharmacy is a professional pharmacy that also provides drug therapy management and DME. A consultation room offers privacy for clinical activities. The residency preceptor is Steve Erickson, Pharm.D., BCPS, who has practiced in the Monroe clinic and hospital pharmacies for over 20 years. Steve was the first recipient of the Innovative Pharmacist of the Year Award by the Washington State Pharmacists Association. Monroe is located near the foothills of the Cascade Mountains in the agricultural Skykomish Valley on scenic Highway 2. Skiing, hiking, backpacking, mountain climbing, and other outdoor activities are available in the nearby Snoqualmie-Mt. Baker National Forest. Monroe is about an hour's drive northeast of Seattle and 30 minutes east of Everett.
Services Offered: Clinic pharmacists have prescriptive authority for initiating new drug therapy, refills, and therapeutic substitution. Because the pharmacy is located in the clinic, the patient's chart is accessible to the pharmacist. Over 200 active patients are enrolled in a pharmacist-managed anticoagulation clinic. Pharmacists prescribe and administer immunizations as well. Clinic pharmacists are also part of the hospice team and have prescriptive authority utilizing clinical algorithms to treat many disorders including pain, anxiety, and sleep management. Many other opportunities exist for the resident. Other areas of involvement include long-term care and home health care. The resident will participate in physician education through regular inservices and newsletters. Physicians and mid-level practitioners frequently consult clinic pharmacists regarding drug therapy issues. Because the pharmacy is a clerkship site for pharmacy students from the University of Washington and Washington State University, the resident will be involved in their educational supervision as well.

The site services a large and diverse community. Started in July of 2005, the program is co-sponsored by the Massachusetts College of Pharmacy and Health Sciences (MCPHS). Opportunities will be available to prepare and provide guest lectures in courses offered at the college. The program will begin the first week of July, and run through the last week of June. Program goals include: (1) providing the resident with the skills needed to establish a clinical practice in a community setting; (2) preparing the resident to assume clinical, educational, operational, and other significant responsibilities within the community setting; (3) providing a mechanism whereby the resident contributes to the development of pharmacy services within the community practice setting; and (4) developing the resident's precepting and teaching abilities. The PCC also functions as a drug information center, providing clinical information to other health care professionals. An extensive reference library is available within the PCC and the resident will have access to the Blais Library at MCPHS. The resident will also be required to provide staff pharmacist services one shift per week.
