Program Description: Four community pharmacy residency positions are available with SUPERVALU Pharmacies (Osco Pharmacy and Cub Pharmacy). Each position is affiliated with a nearby College of Pharmacy. The community pharmacy site, the SUPERVALU corporate setting and the University provide a wide array of opportunities to ensure the resident’s thorough professional development. The resident will engage in a range of activities that develop leadership and teaching (both didactic and experiential) skills. Through active participation in educational opportunities both in the community and academic settings, graduates of this program will have the management and clinical skills necessary to operate a profitable pharmacy while providing financially sound patient care and disease management services.

Program Description: Kings would like to expand its opportunities by offering their resources to a one-year community pharmacy residency program. The resident will develop in-depth training in a wide variety of pharmacy practice areas with a focus on out-patient pharmacy. The resident and the preceptor will develop and implement specific projects to provide learning experiences consistent with the program. Focused Experiences: Residents development and promotion of pharmaceutical care and disease state management services in a community pharmacy. Areas of foucs: Hepatitis C, Diabetes and Long term care. They will be responsible for providing continuity of care to Kings patients by rotating through various local clinics and hospitals, learning about community pharmacy management and operations. Students will also have the opportunity to gain experience in drug compounding of oral, topical and injectable medications.

Program Description: This residency provides an opportunity to practice at Aurora St. Luke's Medical Center Outpatient Pharmacy, a community pharmacy setting within a large tertiary medical center. Residents will concentrate on enhancing existing services and implementing new pharmacy-based medication therapy management programs. Patient care services include comprehensive medication reviews, immunization services, smoking cessation management, transplant medication refill services, and osteoporosis screenings. Residents will also have opportunities to provide medication therapy management services in several clinic settings including family practice and anticoagulation. Coordination of patient care between the pharmacy and clinics will be emphasized. Residents will also gain experience in practice management and professional/leadership skills development. There is the opportunity to precept students and provide ACPE educational programs. The experience is customized to each resident.

Program Description: Campbell University School of Pharmacy, the first accredited community pharmacy residency program in North Carolina, has four positions available. Our sites offer geographic as well as practice diversity. Two practices are in rural communities of about 3000 people each (Benson and Nashville), one is in an urban setting (Durham), in the middle of “The Triangle” of Raleigh-Durham-Chapel Hill, and one is in the beautiful mountain setting of Asheville. Encouragement and support throughout the residency is promoted by close association with all Campbell residents and faculty in monthly residents’ seminars and team-teaching. In addition, each resident will have clerkship precepting responsibilities and complete a major project. Each of our four community sites offers exceptional clinical services allowing the resident a wide variety of experiences in either independent or chain pharmacies.

Program Description: CUMC Pharmacy is located on the ground level of the Creighton University Medical Center in Omaha, Nebraska. It is a community pharmacy which provides outpatient services to patients in the community, CUMC discharge patients, ambulatory clinic patients and university employees. Program Description: Dominick’s Pharmacy offers 2 ASHP-APhA accredited PGY1 residency positions with an emphasis in Community Care. Dominick’s, a Safeway company, operates 77 grocery chain pharmacies in the Chicagoland area. The goal of the Dominick’s Pharmacy Community Pharmacy Practice Residency is to educate and train a pharmacist capable of independently providing pharmaceutical care in the community with complementary abilities and skills to innovate, market, and cost-justify contemporary patient are services. Each position is affiliated with a local college of pharmacy, University of Illinois at Chicago and Midwestern Chicago College of Pharmacy, respectively. Previous work or clerkship experience in providing pharmaceutical care in the community is desirable, but not required. Established in 1996, the education and training experience is designed to develop advanced knowledge and skills in the delivery of pharmaceutical care. Program Description: The Community Pharmacy Practice Residency Program at Evanston Northwestern Healthcare (ENH) is a unique and comprehensive community pharmacy residency program designed to provide exposure to both patient care and management. The goal of this program is to train a pharmacist to develop and implement advanced patient care services in a community pharmacy setting. The resident will conceptualize, integrate, and transform accumulated experiences and knowledge into improved patient outcomes. In addition, the resident will develop pharmacy management skills and professional and leadership skills necessary for advancing pharmacy practice.

Program Description: Fred Meyer Pharmacy has over 125 locations in Oregon, Washington, Idaho and Alaska, and is a division of the Kroger Co., one of the largest grocery store chains in the nation with locations in 32 states. Fred Meyer is a family friendly, One-Stop Shopping store with grocery, apparel, electronics and much more. Most pharmacies have private consultation areas to enhance pharmaceutical care services provided. The resident will primarily work from the Healthy Options Patient Care Center located in Tualtain, OR. Although the residency is based out of one location in suburban Portland, the resident spends time at other Fred Meyer locations including the Healthy Options Patient Care Center at the Main Office in Portland, the Oregon State College of Pharmacy in Portland and Corvallis and other clinics such as Benton County Community Health Center and Central City Concerns Clinic.

The advantage to working with Fred Meyer Pharmacy is the opportunity to implement programs throughout the entire company and potentially influence the practices of several different pharmacies. Program Description: We have two geriatric community pharmacy practice residency positions available; one in a large national chain pharmacy and one in an independent pharmacy located in a retirement community. The structure and the learning objectives for both of these positions will be similar. This residency program is unique in that it will provide an innovative skill set to the residents that will make them extremely marketable in community pharmacy practice, consultant pharmacy practice, industry, and academia. The residents will learn about the special medication needs of the elderly population and how to develop and provide services to improve outcomes from pharmacotherapy. Each resident will be involved with the development of specialty pharmacy services, such as comprehensive drug risk assessments for the elderly and disease management. The residency curriculum will include marketing and reimbursement for pharmacy services, geriatric and establishing collaborative practices.

Benefits Provided: The stipend for this residency is $31,920 for first year residents and $32,920 for second year residents. Program Description: Hartzell’s Pharmacy/Wilkes University residency program is a PGY-1 pharmacy practice residency with an emphasis in community care, located in an independent community pharmacy setting. The resident will participate, develop and refine patient care services including immunizations, medication therapy management, health and wellness screenings and community outreach education. Program Description: Kerr Drug / Wingate University Community Pharmacy Residency is based out of one Kerr Drug community pharmacy site. This site focuses on the development, implementation, maintenance, and marketing of innovative pharmacy services. Residents provide management of diabetes mellitus, heart failure, dyslipidemia, asthma, hypertension, depression, women's health, medication therapy management and immunization services. Other responsibilities include: student preceptorship, practice management, public health education, limited traditional staffing, and clinical research. Multiple elective opportunities also exist. The resident also becomes a member of the clinical teaching faculty at the School of Pharmacy. Residency begins July 1 and end on June 30 of the following year.

Program Description: A one-year accredited community pharmacy residency is offered by the Department of Pharmacy at Centro de Salud Familiar La Fe, Inc. and the University of Texas ElPaso/Austin Cooperative Pharmacy Program. The experiential component is based primarily at Centro de Salud Familiar La Fe, Inc. and its’ clinics, along with other sites that are available with preceptors meeting expected criteria. This residency offers the opportunity to provide pharmaceutical care in the community pharmacy setting, and rotations will be adjusted to maximize the residents learning experience, taking into consideration the strengths, weaknesses and interests of the resident as well as the needs of the program. The Residency Learning System (RLS), developed by the American Society of Health-System Pharmacists (ASHP), is used as the format for this residency program.

Program Description: The community practice residency at Walgreens Patient Care Center is a post-graduate educational and training experience designed to develop pharmacists with expertise in community pharmacy practice. The program is academically affiliated with and financially supported, in part, by the Massachusetts College of Pharmacy and Health Sciences (MCPHS), Worcester/Manchester. This affiliation affords the resident opportunities to develop teaching skills through the provision of experiential education and didactic instruction of pharmacy students.

Benefits Provided: Each resident will be paid a stipend of $50,000 with two weeks paid vacation. Enrollment in the program qualifies the resident for forbearance of most student loans while enrolled in the program. The resident will also be covered under the comprehensive benefits plan in accordance with the Walgreen Company policies.

Selection Criteria: The applicant must possess a doctor of Pharmacy degree from a school or college accredited by the American Council on Pharmaceutical Education, or the equivalent of such, and must hold or be eligible for pharmacy licensure in the Commonwealth of Massachusetts.

Program Description: This is a one-year, post-graduate Pharmacy Practice residency with an emphasis on Community Care. Program Description: The residency offers a unique and challenging post-graduate training opportunity in community care. The program is designed to develop future leaders in community pharmacy practice that are well qualified to provide sophisticated patient care services, develop new cognitive services, and teach pharmacy students in the patient care and classroom environments. Program Description: Advantage Healthcare and Price Chopper Pharmacy are affiliated with the University of Missouri-Kansas City to offer 2 PGY-I Community Pharmacy Residencies. Advantage Healthcare is a licensed consulting pharmacy associated with 2 independent pharmacies. Price Chopper Pharmacy is a family-owned grocery store chain. This 1-year post-graduate experience allows residents to become confident practitioners with effective and innovative leadership skills who advance the profession and improve community health by providing superior patient care in a financially sustainable manner. Both residency sites are advanced community rotation training sites, have access to drug information resources, and participate in local, state and leadership activities. University faculty are an integral piece of the residency training and offer mentorship. Residents have the opportunity to precept rotation students and deliver didactic lectures.

Program Description: UNC-CH Community Pharmacy Residencies are based in 6 community pharmacy sites. All focus on the development, implementation, maintenance, and marketing of innovative pharmacy services. Practice settings include independent and chain pharmacies, primary care clinics, and long-term care facilities. Residents provide management of diabetes mellitus, heart failure, dyslipidemia, asthma, hypertension, anticoagulation, women's health, medication therapy management and immunization services. Program Description: The community pharmacy practice residency is a twelve-month educational and training experience designed to develop advanced knowledge and skills. A strong emphasis placed on ambulatory patient care and the development and provision of innovative pharmaceutical care services in 6 independent and 1 chain pharmacy location. Program Description: The community pharmacy resident will have a variety of opportunities in delivery of pharmaceutical care. Facilities include: the University of Toledo College of Pharmacy, the Kroger Company, the Neighborhood Health Association, including the Cordelia Martin Health Center in Toledo, Ohio. The University of Toledo College of Pharmacy currently partners with Kroger on a project to develop and implement advanced pharmaceutical care services in the retail practice setting. Initiatives at this time are primarily focused upon asthma and diabetes care. The College of Pharmacy also provides pharmacy services for the Neighborhood Health Association and is funded through an HRSA grant that also includes the Family Medical Center of Michigan, Inc. to demonstrate the value of pharmacist delivered care to positive health outcomes. The latter two organizations provide community support and health services for underserved individuals.

Program Description: The goals of the residency program are to advance the Resident Pharmacists' education and training in drug therapy, disease state management, communication skills and to develop and implement innovative patient care programs in Bartell Drug Locations. Residents are involved in providing direct patient care services in medication therapy management, chronic disease monitoring, health screening, travel medicine, and immunizations. Residents gain experiences and develop skills in corporate business administration and operation, as well as prepare a major research project, give in service presentations to staff and administrators, promote patient services, and attend several College/School of Pharmacy career fairs. The strong affiliation with the UWSOP allows for guidance, mentorship, and teaching opportunities with university faculty.

Benefits Provided: The Resident Pharmacist will be an employee of the Bartell Drug Company and receive company benefits. Additionally, residents will be eligible for appointments in the UW SOP Dept of Pharmacy as Clinical Instructors. Stipend: The Resident Pharmacist will receive a stipend of $50,000 for the 12-month program. Benefits: The Resident Pharmacist will receive 2 weeks paid vacation (times to be agreed upon by the resident and the preceptor). Medical benefits will be provided through Bartell Drugs, and will start the first of the month after employment. The Resident Pharmacist will be reimbursed for travel and expenses (in compliance with the Bartell Drugs Travel Expense Policy) to attend professional meetings and presentations. Travel reservations and expense reports must be approved by the Bartell Drugs Director of Pharmacy.

The residencies are offered at various independent community pharmacy sites, mass-merchandiser, and local and national supermarket chain pharmacies.

Program Description: The community practice residency at Walgreens Patient Care Center is a one year post-graduate educational and training experience designed to develop pharmacists with expertise in community pharmacy practice. The program is academically affiliated with and financially supported, in part, by the Massachusetts College of Pharmacy and Health Sciences - BOSTON (MCPHS). This affiliation affords the resident opportunities to develop teaching skills through the provision of experiential education and didactic instruction of student pharmacists.

Benefits Provided: Each resident will be paid a stipend of $50,000 with two weeks (10 days) paid vacation. The resident will also be covered under the comprehensive benefits plan in accordance with the Walgreen Company policies.

Selection Criteria: The applicant must possess a Doctor of Pharmacy degree from a school or college of pharmacy accredited by the American Council on Pharmaceutical Education, or the equivalent of such, and must hold or be eligible for pharmacy licensure in the Commonwealth of Massachusetts. Once applications are received, the candidates will be invited on site for a formal interview at MCPHS as well as the Walgreens Patient Care Center.

Program Description: The goal of this jointly sponsored one-year residency program is to enhance current community pharmacy practie adn to evaluate the effectiveness of a continuum of care in this setting. The resident will engage in a variety of integrated experiencee including rotations at Rite Aid Corporate headquarters, academic and corporate drug information centers and the community pharmacy environment. While the residency emphasizes diabetes education, it also encompasses comprehensive pharmaceutical care at Rite Aid pharmacies, WVU outpatient operational and financial management skills.
