Delivering Medication Therapy Management in the Community is an innovative and interactive training program that explores the pharmacist’s role in providing MTM services to patients. Pharmacists have a tremendous opportunity to receive reimbursement for monitoring and improving medication use in patients with complex medication regimens. The program teaches pharmacists the essential skills necessary to become a successful MTM practitioner. This certificate program will enhance pharmacists’ clinical expertise in evaluating complicated medication regimens, identifying medication related problems, and making recommendations to patients, caregivers, and health care professionals. Delivering Medication Therapy Management in the Community is conducted in two parts:

  • A self-study learning component consisting of 4 modules and practice interviews with patients
  • A live interactive training seminar consisting of case studies and hands-on patient interviews and assessment practice sessions

The goals of the program are to help participants:

  • Obtain the clinical knowledge, skills, and motivation needed to establish MTM services for patients with complex drug regimens
  • Learn to conduct a thorough patient medication history, complete a personal medication record, and develop a medication action plan
  • Identify patient-specific medication related problems and address these concerns by establishing individualized medication treatment regimens
  • Document and bill services appropriately

Completion Information:

  • Successful completion of the self-study learning component will result in 10 hours (1.0 CEU) of continuing pharmacy education (ACPE UPN: #202-999-07-089-H04).
  • The live training seminar is approved for 8 hours (0.8 CEU) of continuing education credit (ACPE UPN 202-999-07-236-L04-P).
  • The post-seminar exercise is approved for 3 hours (0.3 CEU) of continuing education credit (ACPE UPN 202-999-08-098-H04-P).
  • CPN - 202-0012. Expiration Date: 03/16/2010.
  • A Certificate of Achievement will be awarded to participants who successfully complete all program components.
