Project ImPACT: Hypertension
Overview
Launched in 2011, Project ImPACT Hypertension is a one year
demonstration project in collaboration with Wayne State University in
Detroit, Michigan. Project ImPACT: Hypertension aims to demonstrate
that improved patient identification and subsequent persistence
(continuing with therapy) and compliance (taking as directed by
physicians) with hypertension therapy reduces risk and adverse events
over time. The initiative is focused around an employer investing in
wellness and changing the way they pay for health management services.
The incentives are aligned for patients and pharmacists to improve
patient outcomes related to medication therapy. Patients are being
provided with wireless at-home blood pressure monitoring devices that
report data to the pharmacist who is managing the patient’s care.
Pharmacists are integrating this information in Medication Therapy
Management services to better monitor and educate the patient on healthy
therapeutic and lifestyle behaviors. The main objectives of this project

Over 90 patients are enrolled and receiving care in the project. This
model involves patients, pharmacists, physicians and other providers
collaborating to improve cardiovascular health.
