MTM occurring across the nation—Interactive map shows you where

Pharmacists wanting to network, providers looking for collaborators, policymakers gauging uptake—everyone finds what they’re looking for on pharmacist.com.

APhA has launched an interactive map that identifies pharmacists who are making a difference in patient’s lives through the provision of medication therapy management (MTM) services. By clicking on a state, the user learns who has been profiled in APhA’s MTM magazine, Pharmacy Today, and can view lists of pharmacies providing these innovative services. Testimonials posted by pharmacists on healthreform.gov are also being added.

Medication therapy management was just a phrase on a piece of paper when Congress passed legislation creating Medicare Part D in late 2003. By the time MTM was implemented in 2006, the pharmacy profession, through an effort led by APhA, had developed a model framework for community pharmacy that was designed to improve care, enhance communication among patients and providers, improve collaboration among providers, and optimize medication use, thereby improving patient outcomes. The current version of the core elements of the MTM service model is version 2.0, and this document guides pharmacists as they seek to make MTM the definitive standard of practice in every city, town, and village across America.

As with most new ideas and products, innovator pharmacists first created MTM services based on previous pharmaceutical care efforts. Early adopters were soon emulating the innovators, and MTM exponential growth is now evident as one explores the pharmacist.com map.
